Mentors - Please clip one of these forms off and fill it out for each game your mentee does with you. Mentees- please make sure that this form is completed by your mentor. Send The completed form to the current Sec/Treasurer and they will send you your check.

Mentors - You should complete print and complete this form for EACH mentee that was assigned to you. This should then be delivered to our current Vice President.
